The FZW Chaos Championship was the top title in the Forum Zone Wrestling fantasy wrestling promotion's Chaos brand. The first champion was determined in a tournament format when Alex Star won the vacant title from Shade in a TLC match. The last champion was "The Son of Anarchy" Jason Russo who won the title by defeating "First Class" Lewis Rivers on January 17, 2016 at FZW: Kingdom Come.
